We’ve designed our boutique membership practice to be affordable to patients who value convenience, personal attention, and proactive wellness support.  

Please note:

  • There are a limited number of membership slots in our practice.   Once these slots are filled, any non-member patients may be asked to see a different provider in our office which may include a physician assistant or nurse practitioner
  • The monthly pricing for our memberships is designed to increase as our membership practice fills up – the earlier a patient joins, the better pricing they receive.  To lock-in the lowest pricing you must enroll right away.  Once locked-in, your membership pricing is guaranteed not to increase for at least 2 years
